Forms can either be turned in at the High School Office or scanned/emailed to Mrs. Wales.
Adult teams only need the "Team Registration" form and $60 to sign up for the tournament.
High School and Junior High Teams need the "Team Registration" form for their Captain AND a "Player Waiver" Form for the rest of their teammates.
